Fortified Health Club: Small Changes Make Big Impacts


Below are two links to your Allostatic Load Surveys. This study is based on monitoring your progression over this 12-week period. The first collection is to see where your baseline levels are, and the second collection is to monitor your progression after making the changes prescribed to you by Dr. Rachel and your experts. Please be honest with all of your answers, and remember, your information is protected and cannot be traced back to you. This is for research purposes and for you to have a tailored protocol for better results. You will not be penalized in any way based on your individual results. These surveys will guide you to your personalized protocol prescribed by Dr. Rachel based on your results.

To complete these surveys, you need to have your blood work results in front of you. You will also need to know your waist and hip measurements or have access to a tape measure while taking this survey. There is a video within the survey to show you how to take these measurements. This survey is not timed, and you should only complete each survey one time. Make sure to select the appropriate survey for the module you are in. If this is your first time doing this survey, please select Survey 1. If this is your second time, please select Survey 2. If you do not have your blood work results, refer to your Direct Labs account to retrieve them, or ask your PCP for a copy of your results (if you ordered your labs through them).
